Vital Records (Births, Deaths & Marriages)
Certified copies of all vital records are available over the counter at the Town Clerk's Office during business hours. Certified copy requests by mail are generally processed on the day they are received. For faster return service enclose packaging and stamps for express mail or another express service.
Births
The Plymouth Town Clerk's Office registers birth certificates only for persons born in Plymouth or persons whose mother's stated residence is Plymouth on the birth certificate.
To order a birth certificate, you must know if the parents of the child on the certificate were married at the time of birth. If the parents were not married, only those persons whose names are on the birth certificate may apply, and those persons must provide a copy of a picture ID.
To order online, please visit the City Hall Systems - Birth Certificate page. For online orders of restricted birth records, it is necessary to attach a photo ID to your order.
To order by mail, please provide the following:
- The child's name and date of birth
- A check payable to the Town of Plymouth for $15 times the number of certificates
- A stamped self-addressed envelope
Marriages
Only couples who applied for their marriage license in the Plymouth Town Clerk's Office prior to marriage may request certified copies of their marriage certificates in Plymouth.
To order a marriage certificate, you must know if the parents of both spouses were married at the time of the birth of the spouses. If the parents were not married, only those persons whose names are on the marriage certificate may apply, and those persons must provide a copy of a picture ID. [NOTE: Couples looking to get married, must apply for a marriage license.]

- The name of one or both spouses on the certificate and the date of marriage.
- A check payable to the Town of Plymouth for $15 times the number of certificates.

Deaths
Certified copies of death certificates are available only for persons who died in Plymouth or whose residence was listed as Plymouth on the death certificate if they died in Massachusetts.
To order online, please visit the City Hall Systems - Death Certificate page. Online orders for certified copies are processed the next business day.
To order by mail, please provide the following:
- The name of the decedent and date of death
- A check payable to the Town of Plymouth for $15 times the number of certificates
- A stamped self-addressed envelope.
