The Town of Plymouth is launching its first-ever Community Health Assessment Survey this Saturday August 23rd at the Annual Waterfront Festival, as part of the inaugural Plymouth Community Health Assessment & Community Health Improvement (CHA–CHIP) Project. Community Health Assessments (CHAs) measure the health-related needs and strengths within a community, while Community Health Improvement Plans (CHIPs) help put CHA data into action by bringing stakeholders together to create a shared roadmap for community health.
